Connected Families: Before-Program Survey
Why we’re doing a survey:
Thank you for your commitment to this program! To help us better understand the impact and effectiveness of this program, we would appreciate your time in completing a few brief surveys (this should take you less than 30-minutes to complete).
At the end of the 6-week program, we will ask you to complete the same surveys.
Once we have received your survey, we will send you a $15 gift card (one for the before surveys and one for the after surveys).
Your survey responses are confidential and the information gathered will be strictly used for program improvement and funding applications.
